Shaping Sheffield: The Plan

You may know that Shaping Sheffield was developed during 2016-17 when more than 60 organisations in Sheffield joined together to commit to a single plan for improving health and wellbeing in the city.

We want a genuine cross-section of staff from across the seven partners which make up the ‘Accountable Care Partnership’ (ACP) to be involved in this work. The partners involved are NHS Sheffield Clinical Commissioning Group, Primary Care Sheffield , Sheffield Children’s NHS Foundation Trust, Sheffield City Council, Sheffield Health and Social Care NHS Foundation Trust Sheffield Teaching Hospitals NHS Foundation Trust, and Voluntary Action Sheffield.

The ACP high-level priorities are:

  • to improve the health and wellbeing of Sheffield residents

  • to reduce health inequalities

  • to focus on ways to keep people healthy, preventing them from becoming unwell, and

  • to develop Sheffield as a ‘person-centred city’

Collectively we have started to achieve so much, but there’s more to do. And we need the help of the public and staff to really transform the health and wellbeing of people in Sheffield. We want to see if the plan is right, find out people’s priorities for the next few years, and get people’s views on how we spend our collective budgets. We want to work with you and your organisation’s staff to do this.
